What this checklist does not do. It contains no electrical, mechanical, adjustment, bypass, or repair instructions. It does not tell you what is wrong or how to fix it. Do not record a working keypad, gate, door, alarm, or other access code anywhere in this checklist or in a service request. A provider evaluates the actual gate and confirms diagnosis, scope, pricing, and terms before work begins.

1. Gate location and type

Property address and gate location (e.g., front driveway, rear alley, parking structure)
Gate movement type (check one)

2. Current position and condition

Gate is currently closed
Gate is currently open
Gate is partially open or stopped mid-travel
Gate is off track, tilted, or no longer supported correctly
Gate moves manually when the operator is disengaged
Gate does not move manually (describe in section 5)
Describe anything visually unusual about the gate's current position

3. Visible operator information

Read labels on the operator housing without opening panels or covers. Do not touch wiring, terminals, or interior components.

Manufacturer name (as shown on the label)
Model number (as shown on the label)
Serial number (if visible on the label)
Power supply (e.g., 120 V AC, solar/battery, 24 V DC — if labeled)
Location of the operator unit (e.g., post-mounted, underground, overhead)
A NRTL certification mark is visible on the operator label (e.g., UL, cUL, Intertek/ETL)
No certification mark is visible
Label is missing, damaged, or unreadable

4. Connected controls

List any exterior controls you can see. Do not record working codes, PINs, or credentials.

Keypad (entry code — do not record the code itself)
Key switch
Push button or intercom at the gate
Telephone entry or callbox
Proximity card or fob reader
Vehicle loop detector (in-ground sensor)
Remote control (transmitter/receiver)
Smartphone app or cloud control
Other / unknown
Which of the above controls still operate the gate correctly (if any)?
Which controls do not respond or behave unexpectedly?

5. Symptoms and recent changes

Describe what you observe from a safe distance. Do not attempt to operate a gate that is damaged, off-track, or behaving unpredictably.

Primary symptom (e.g., gate does not open, reverses before fully open, makes grinding noise, moves slowly)
When did the symptom first appear?
Does the symptom occur every time, sometimes, or only under certain conditions?
Any recent events that preceded the symptom (e.g., storm, power outage, vehicle strike, new wiring, remote change)?
Control panel shows an error code or flashing light — note the code or pattern
Operator makes noise (describe: grinding, clicking, humming, none)
Gate reverses during travel without hitting anything visible
Gate stops at the same point every time
Gate moves in one direction but not the other
Gate moves only on constant-pressure (must hold button)
Safety sensor indicator light is on or flashing

6. Visible entrapment-protection devices

Note only what is visible from a safe distance without touching any equipment.

Photoelectric (beam) sensor visible on one or both sides of the gate opening
Contact edge or sensing strip visible on the gate leaf
Warning signs or decals visible on or near the gate
No external sensors visible
Cannot determine from safe distance
Note the approximate location of any visible sensors (e.g., post at 18 inches, leading edge of gate)

7. Safe photos to take

Take photos only from outside the gate opening and gate path. Do not stand in the path of a moving gate. Do not open panels or covers to photograph interior components.

Full view of the gate from outside the property — gate in current position
Operator housing label (manufacturer, model, serial — readable without opening panels)
Operator mounting location (post, underground housing, or overhead)
Gate track, rollers, or hinge visible from outside
Visible sensors or devices and their mounting location
Any visible damage to the gate, post, track, or operator housing
Error code or indicator light on the control panel (if readable from outside)

8. Service history

Approximate age of the gate operator (if known)
Date and description of most recent service or repair
Name of provider who performed the last service (if known)
Any warranty, service contract, or permit documentation on file?
Other relevant history

9. Questions for the provider

Write your questions before the appointment so nothing is forgotten.

What is the diagnosis and what work is being proposed?
Is a permit required for this scope of work?
What is the written estimate for parts and labor?
Is the provider licensed with the Arizona Registrar of Contractors?
How long will the repair or replacement take?
Will the existing entrapment-protection devices be tested after work is complete?
What warranty covers parts and labor?
Is any structural, electrical, or excavation work included in the scope?
